This will cause major damage to your door and the surrounding body. The car door will never close again without major body repairs.
So, it really doesn't matter where you put the crowbar. Put it in the jamb just below the window level and pry away until the door opens. Again, you will cause MAJOR damage to your car doing this. The door will NOT close again!!!!.
If you are trying to access the car because you don't have a key and you don't want to call a locksmith for some inexplicable reason, break a window. They are much cheaper than body repairs.

Ok here's what you do.
1) Step one, murder a tow-truck driver and steal his tow-truck. This should be easy enough to accomplish, those tow-truck idiots are always towing perfectly good cars...needless to say, he or she had it coming.
2) Tow your crappy car with the broken door to an ethnic neighborhood. Make sure there are plenty of minorities walking around (look for baggy clothing and brownish skin) *this is key*. Then leave your car parked next to a curb.
3) Drive away and wait for it to get dark, (this is when the magic happens!).
4) Return to your car in the morning and i'm sure you will be very pleased with the result. A) Your car door(s) should be open or completely removed entirely. B) All of your junky stereos and speakers will be removed for your convenience (lets face it, if you are prying your door open with a crowbar, i'm assuming you have shitty things to begin with and are due for an upgrade!). C) You may even be lucky enough to discover some fun drugs or cheap liquor that your special night helpers left behind.
Rest assured, this is at least a 5x better solution than whatever you and your peanut brain were planning with a crowbar.

Hopefully this is to gain access to your own vehicle, and although i havent crowbar'd a door before, or even held a crowbar, id imaging the best pivot point would be from the top and just use force?

I agree with Rolo. Will be cheaper to bust in the window than fix the messed up door.

It will be cheaper to bust a door window out repair cost wise - if it's a house, same thing.
